How to Request Equipment Service

How to Request Equipment Service on the MEDILOGIX Customer Portal

If you need equipment service, please follow the steps below:

  1. Log into the MEDILOGIX Customer Portal with your username and password
  2. Go to the Orders tab and click Request Service.
  3. Complete the Request Equipment Service form.
  4. Fill in all information requested (* indicates required information)
  5. Please type any comments that will help us service the equipment.
  6. Click the Submit Request button.
